Volunteers under the age of 18 require written permission from a parent/guardian to volunteer.

This applicant has my permission to serve as a Volunteer at Parkview Home. I have read the Volunteer information available and understand the responsibilities and commitments my child has made to Parkview Home as a Volunteer. I understand that Parkview Home will not be held liable for any accidents/incidents that may occur on the premises.

Criminal Reference Checks

Volunteers under the age of 18 are exempt.

In submitting this application I understand and accept that if I am offered a position at Parkview Home, confirmation of permanent employment or volunteer status with Parkview Home is conditional on my submitting a copy of a Police report indicating a successful Criminal Reference Check as relates to the position being offered. This is the only purpose for which the Criminal Reference Check will be used and all such information will remain confidential. The appropriate form will be made available to you, and will be signed by the authorized Parkview Home representative. The results of the Criminal Reference Check must be presented to the hiring supervisor within thirty days of any conditional offer of employment. The cost of the Criminal Reference Check is to be borne by the applicant.

POLICY: “Steps will be taken to protect the safety of Parkview Home Residents, Staff and Home Property. A final decision regarding the hiring of new employees and volunteers is conditional upon a satisfactory criminal reference check.”


Mandatory Health Screening

In submitting this application, I understand and accept that if I am offered a position at Parkview Home, confirmation of permanent employment or volunteer status with Parkview Home is conditional on my HEALTH SCREENING RESULTS.

Under Section81 of the Fixing Long‑Term Care Act, 2021, every licensee (i.e. long‑term care home operator) must ensure that screening measures are conducted—per regulations—prior to hiring staff or accepting volunteers

Ontario Regulation O.Reg.246/22 clarifies that these screening measures include health screening requirements like TB screening

The Two-Step Tuberculin Skin Test (TST) is a method used to detect latent tuberculosis infection (LTBI), especially important for people who will work or volunteer in high-risk environments—like long-term care homes.


Volunteer Agreement

As a Volunteer, I accept the responsibility to maintain my knowledge / understanding of my volunteer role. I commit to participation in training and evaluation of activities. In the event that my Volunteer involvement is not compatible with Parkview Home requirements, then the decision of the Director of Program and Support Services will be final (i.e. retaining, transfer to another area or termination).
